Responsibilities of the Clinical Documentation Specialist include:
- Facilitating improvement in the overall quality, completeness and accuracy of medical record documentation
- Obtaining appropriate clinical documentation through extensive interaction with physicians, nursing staff, other care team members, and medical records coding staff
- Educating all members of care team on documentation guidelines on an ongoing basis
- Demonstrating knowledge and skills necessary to provide care appropriate to the age of the patients served
- Demonstrating knowledge of the principles of growth and development over time
- Maintaining quality admission reviews
- Completing follow-up of admission reviews
- Supporting the growth and development of care team members
- Participating in performance improvement activities
- Providing care that reflects an understanding of individual cultural and religious diversities and needs as well as recognition of patient rights

4+ years of clinical experience
3-5 years of acute care experience
2+ years of case management or performance improvement experience
Knowledge of discharge planning, ICD-9 coding, utilization review, quality management, data analysis, managed care, and management
Computer proficiency
Valid New York State Registered Nurse (RN) licensure
Bachelor’s degree in Nursing (BSN)
