A multi-service, not for profit agency that provides a continuum of mental health, social services and housing to persons living with HIV/AIDS and/or mental illness, and substance abuse is seeking to hire a Nurse Practitioner. The individual in this position will be responsible for developing protocols to integrate physical health activities into a mental health setting.

Health care

As the Nurse Practitioner, you will be responsible for:

• Conducting initial assessments with patients and formulate health goals to be integrated into the comprehensive treatment plan and treatment plan reviews for each participating client
• Making referrals to specialists as necessary
• Collaborating with other providers and coordinating care for participating clients
• Developing strategies to educate mental health staff on the need for and goals of the integrated healthcare program
• Developing weekly group sessions in diabetes, hypertension, smoking cessation and other behavioral health issues
• Conducting monthly follow-up appointments with patients to chart their medical status, including pharmaceutical adherence and behavioral health changes
• Maintaining patient files, with the assistance of support staff

• Masters in Nursing with certification as a Nurse Practitioner
• Community minded, patient, creative, flexible, compassionate, and culturally sensitive to persons who are severely and persistently mental ill
• Desire to work in a community based setting and ability to work as part of a multidisciplinary team

• Bilingual in Spanish and English
