The Occupational Therapist will evaluate participants' level of function to establish treatment programs to prevent disability by helping participants to maintain and/or restore physical potential by means of such modalities as training, exercise, orthotics and assistive devices. The individual is responsible for conducting initial assessments and periodic reassessments, plan of care, coordination of 24 hour care delivery, regularly informing the interdisciplinary team (IDT) of the medical, functional, and psychosocial condition of each participant, remaining alert to pertinent input from other team members, participants and caregivers, as well as documenting changes in a participant's medical record consistent with documentation policies.

• Current New Jersey Occupational Therapy license
• Certification with the National Board for Certification in Occupational Therapy (NBCOT)
• Current driver's license with proof of insurance
• Knowledge of gerontology and enthusiasm for treating the geriatric population
• Knowledge of common safety hazards in the home and improvements to establish a safer environment
• Ability to work as a member of a multidisciplinary team in a community based setting
