Responsibilities of the Outpatient Nurse Coordinator include:

• Reviewing requests for referrals for appropriate level of care
• Seeking additional information from providers regarding requests
• Referring requests not within approved authority to medical directors for review and decision
• Monitoring the appropriate level of care, utilization of resources and continued needs for the patient/member across the continuum of care
• Assisting physicians, care providers and patient/family in understanding benefits
• Determining eligibility of benefits

• 4+ years of clinical experience
• Previous experience in a managed care company in either utilization review or case management
• Computer proficiency
• Valid New York State LPN licensure
• Excellent communication and interpersonal skills
